Google Forms

We try to make permission slips and form filling as easy as possible for our parents. This year we are using Google Forms to collect information. These online forms work on all devices and should be a lot less time consuming for parents. They also don't rely on the children to remember slips and forms...something children often forget in our experience!

 

 

We will try to include relevant links to Google Forms on this page, so that parents can easily find what they are looking for, especially if you've accidentally deleted the emailed link!

 

 

The forms below (A - E) must be completed by all parents new to St Joseph's.

Form Z should be completed by existing parents when any contact details change.
